This article examines the critical aesthetic, functional, and psychosocial considerations surrounding nipple–areolar complex (NAC) reconstruction following mastectomy, an often underappreciated but pivotal component of breast reconstruction. Drawing on parallel surveys of post-mastectomy patients and experienced plastic surgeons, the authors explore the degree to which current nipple reconstruction techniques align with patient expectations for natural appearance, projection, sensation, symmetry, and long-term durability. The findings highlight a consistent gap between desired outcomes and what existing surgical and tattoo-based approaches can reliably deliver, underscoring the profound impact of NAC reconstruction on body image, self-esteem, sexuality, and overall quality of life. By synthesizing patient-reported priorities with surgeon-reported limitations, the article frames the clinical need for more durable, biologically integrated solutions that better address both aesthetic and psychological recovery after mastectomy.
